SOCSO - Borang 2 Form (new employee registration form)

For the purpose of registration with SOCSO, an employer is required to complete the 'Employee’s Registration Form' (Form 2). This is required regardless of whether the employee has ever registered and made contributions to SOCSO while working at a previous employer.

This monthly report shows SOCSO data for all new employees that were hired during the month. 

You are able to filter the report using the following:

  • As at month: You can change months by clicking on the calendar icon;
  • Pay schedule: Will list all active pay schedules within the business;
  • Employing entity: Only applicable if there are multiple employing entities set up;
  • Employee: An individual employee can be selected;
  • Employee default location: As configured on the Employee file > Pay run defaults page.

Once you have set the desired parameters, click the 'Generate' button to show the snapshot view results of the report.

Then clicking on the 'PDF' button will generate the SOCSO form 2. The data that is reported on includes:

  • Kind of introduction (eg passport)
  • Date of birth
  • Employee name (as on identity card)
  • Gender
  • Nationality
  • Work commencement date
  • Whether or not employees earn greater than RM3,000 per month
